Community Adult Weight Management Programme
Residents of Preston, Chorley and South Ribble now have access to an exclusive new service designed to improve the health and wellbeing of the local community. ABL’s experienced group of Lifestyle Coaches are here to help clients with a BMI between 25 and 35. Reviewing eating habits with information on how to maintain weight loss through lifelong lifestyle and behavioural change, this programme looks at strategies for long term weight management. Sessions are designed to include a blend of nutrition advice and activity to suit the needs of the group. Focusing on breaking down some of the barriers that in the past may have prevented people from achieving their goals, we aim to move clients away from the confusing messages of the quick fix fad diet and focus them on setting realistic and achievable goals in a none restrictive way. Group sessions will be held at community venues throughout the week offering morning, afternoon and evening sessions making us as accessible as possible.

FAB (Food, Activity, Balance): Family Weight Management Programme
These services are for children with BMI’s >91st centile and their families. The service offers weekly education and activity sessions focusing on topics such as food labelling, healthy meals, portion sizes and goal setting. Weekly physical activity sessions are also included. The FAB service does not endorse dieting or restricting certain foods, it is about education and empowering families and children to make their own educated choices.

Active Lives: Physical Activity Service
This FREE physical activity service is available to anyone who does less than 150 minutes’ activity each week. This service runs for 12 weeks and aims to help support clients to increase their weekly activity, using classes such as circuits and chair based exercises, health walks and supported gym activities.

Active Lives: Exercise on Prescription
ABL Central Lancs will be providing a free 12-week GP Referral Scheme for residents in the Preston, Chorley and South Ribble area.
Residents are able to access the GP Referral Scheme provided they have a health condition (High B.P/Diabetes/Bone or Joint problems etc.), and are currently inactive (Less 150 minutes Physical Activity per week).
Resting Systolic B.P > 180mmHg
Resting Diastolic B.P > 100mmHg
Uncontrolled Tachycardia > 100bpm
Unstable or Acute Heart Failure
Recent Embolism; Thrombophlebitis
Active Pericarditis or Myocarditis
New Angina diagnosed < 6 weeks – Investigations must be complete
Awaiting further Cardiology investigations – unless permission to exercise has been given by Cardiologist/Phase III Rehab staff/GP
Severe Mental Health disorders
Falls requiring specialist need
Those accessing the scheme will have access to specialist GP Referral instructors who will provide;
1-1 consultation to discuss individual needs
Specific, tailored physical activity programme
Regular reviews at 3, 6, 9 and 12 weeks, plus 6 and 12 months
Strategies to overcome barriers and maintain long-term adherence
HOW TO REFER?
Those residents who meet the criteria for the scheme can be referred by their GP, Practice Nurse or other Health Professional. Referrals can be made using the referral form and should be faxed or sent via post.
Residents wishing to access the scheme can also self-refer. However, GP clearance will be sought by the Active Lives and Healthy Weight team before the resident can commence any physical activity if they have a health condition.
